Job Title: RN Case Manager – Acute Transitional Care (WellMed, Telecommute)
Please note these are remote but candidates would need to be in the Treasure Coast, Tampa, Citrus, Northeast Florida, or Orlando area.
Schedule: Mon–Fri, 8am–5pm EST
We are seeking an RN Case Manager to join our Comprehensive Clinical Management Department at WellMed. This telecommuting role ensures smooth transitions of care across inpatient and outpatient settings using clinical expertise, care coordination, and effective resource utilization to improve patient outcomes.
As an RN Case Manager, you will work closely with primary care providers and interdisciplinary teams to guide patients through the continuum of care. Responsibilities include patient assessments, discharge planning, motivational interviewing, and ongoing care management both remotely and in-person when needed.
Key Duties:
Support transitions from acute care, SNFs, and ambulatory settings
Coordinate discharge plans and follow-up care
Communicate with providers, hospitals, and families
Educate patients about resources, care plans, and benefits
Document care using electronic platforms
Provide remote clinic coverage and travel as needed
Qualifications:
RN license in Florida
Bachelor’s or Associate’s in Nursing (with 4+ years' experience)
2+ years clinical experience (hospital, clinic, or physician office)
Preferred:
Care management, utilization review, or discharge planning experience
HMO background and CCM certification
This RN Case Manager position requires strong communication, leadership, and critical thinking skills. Familiarity with value-based care, HIPAA, and patient-centered care practices is essential.
