Senior LTSS Service Care Manager (RN) Compensation
- The pay for this position is $42-$46 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Senior LTSS Service Care Manager (RN) Highlights
- The required availability for this position is Monday–Friday, 8:00 AM–5:00 PM
- Remote position – candidates must reside in Texas, preferably near the Abilene or San Angelo area
- Field-based, member-facing position with 3 days of member visits and 2 days of documentation per week
- Possibility to extend or convert after 6-month assignment
Senior LTSS Service Care Manager (RN) Responsibilities
- Assists with developing and continuously assessing ongoing long-term care plans
- Coordinates care between members, families/caregivers, and provider teams
- Monitors care plans, member status, and outcomes to identify appropriate interventions
- Conducts STAR Kids Screenings, Individual Service Plans, and required service forms
- Performs home/site visits and telephonic follow-ups
- Documents all care management activities to ensure regulatory compliance
- Educates members and caregivers on services, options, and benefits
- Collaborates with healthcare partners and leadership to enhance care quality
Senior LTSS Service Care Manager (RN) Requirements
- Registered Nurse (RN) licensure in the state of Texas or Compact State Licensure
- Graduate from an accredited school of nursing (ADN required; BSN preferred)
- 4–6 years of related experience in case management or long-term services and supports
- Must obtain Resource Utilization Group (RUG) certification within 90 days of hire
- Strong organizational and time management skills
- Must be comfortable with field visits and member-facing responsibilities
Senior LTSS Service Care Manager (RN) Preferences
- BSN preferred
- Bilingual English/Spanish a plus
