Job summary

CURA-HPC EASTOK LLC is seeking a dedicated RN for hospice care in Sallisaw, Oklahoma. The role involves triaging after-hour calls, visiting patients, and communicating outcomes with families and staff.

You will administer medications and treatments per hospice guidelines, develop and document Plans of Care, and ensure accurate patient status updates in the chart. Prior hospice or Medicare experience is valued.

Qualifications

• Graduate of an accredited school of nursing (ADN-RN or BSN).
• 2 years nursing experience preferred.

Responsibilities

• Triages after hour calls and directs patient care.
• Completes visits, documents outcomes with patients and caregivers.
• Implements Plan of Care per physician orders and hospice guidelines.
• Administers medications and treatments according to policy.
• Maintains accurate patient status documentation.
• Evaluates Medicare eligibility and admits qualifying patients.

Completes visits to address identified issues, performs interventions and communicates instructions and outcomes with patients, caregivers and staff.Demonstrates the ability to transfer scientific knowledge from social, biological and physical sciences in applying the nursing process.Demonstrates knowledge and competency in performing baseline-nursing skills. Maintains clinical competencies according to Hospice standards.Evaluates and admits only qualifying patients according to Medicare eligibility requirements.Administers all medications and treatments according to Hospice policy and procedure, accepted standards of care and Hospice guidelines.Communicates effectively with patients, families and physicians.Performs accurate assessments, develops and implements a written Plan of Care with the patient, following physician's orders and according to Hospice guidelines. Performs and documents nursing interventions including patient education according to the Plan of Care and adapts these actions as patient needs change.Documents patient status in an accurate, timely and concise manner as required by Hospice policy.
