Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1, whole-person care in patients' homes. You will coordinate individualized care plans with an integrated care team and support families through complex decisions with empathy and professionalism.

We value autonomy in practice, assess needs, educate patients and caregivers, and participate in on-call rotations within a compassionate team dedicated to dignified end-of-life care.

Qualifications

• RN required with 1+ years acute care experience.
• Hospice or home health experience preferred.
• Ability to develop individualized care plans with patients and families.
• Educate patients and caregivers on care techniques and medications.
• Rotate on-call duties and coordinate seamless care transitions.
• Must have reliable transportation and a valid driver’s license.

About the role

In the role of Registered Nurse Case Manager, you’ll be entrusted with providing 1:1, whole-person care to patients and families in their homes. Partnering with an integrated care team, you will coordinate individualized care plans, support families through complex decisions, and offer comfort with professionalism, empathy, and respect.

Qualifications

• Registered Nurse Case Manager Requirements & Responsibilities:
• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Complete initial and ongoing comprehensive assessments to determine hospice needs
• Develop individualized care plans with the patient and family, updating as needs evolve
• Administer medications and treatments as prescribed
• Educate patients and caregivers on care techniques, medications, diet, and symptom management
• Rotate on-call duties and ensure seamless care transitions
• Must have reliable transportation, valid driver’s license, and proof of auto insurance

The starting salary range for this role is $ 85,000 - $97,000. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
