Job summary

St. Croix Hospice in Wisconsin is seeking a Registered Nurse Case Manager to provide 1:1, whole-person care to patients and families in their homes.

You will collaborate with an integrated care team to coordinate personalized care plans and support families through complex decisions with professionalism, empathy, and respect. The role requires an RN with an associates, bachelor’s, or master’s degree and at least 1 year of acute care experience, hospice/home health preferred.

Qualifications

• 1+ years of acute care experience.
• Hospice/home health experience preferred.
• Must have reliable transportation and valid driver''s license.

Responsibilities

• Perform 1:1 whole-person care at patients' homes.

• Educate patients and caregivers on care techniques and medications.
• Rotate on-call duties and manage transitions.
• Administer medications and treatments as prescribed.

In the role of Registered Nurse Case Manager, you’ll be entrusted with providing 1:1, whole-person care to patients and families in their homes. Partnering with an integrated care team, you will coordinate individualized care plans, support families through complex decisions, and offer comfort with professionalism, empathy, and respect.

Qualifications

Registered Nurse Case Manager Requirements & Responsibilities:

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Complete initial and ongoing comprehensive assessments to determine hospice needs
• Develop individualized care plans with the patient and family, updating as needs evolve
• Administer medications and treatments as prescribed
• Educate patients and caregivers on care techniques, medications, diet, and symptom management
• Rotate on-call duties and ensure seamless care transitions
• Must have reliable transportation, valid driver’s license, and proof of auto insurance

"The starting salary range for this role is $87,000-$95,000 annual base salary. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.

A 5K sign-on bonus is available for newly hired employees.
