RN Case Manager

StartDate: 5/19/2025 Pay Rate: $2000.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 1+ year recent acute hospital case management experience

PREFERRED QUALIFICATIONS – Interqual & Meditech Experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8A-4:30P. Every other weekend & holidays in rotation.

SYSTEMS – Interqual/Meditech

START DATE – ASAP
