Job summary

Friends Hospital in Philadelphia, PA, is seeking an experienced RN Case Manager for a full-time Day shift to coordinate inpatient case management, facilitate communications with social workers, referral sources and service providers, and support safe, timely discharge.

You will review documentation, coordinate bed placement, guide patients through initial treatment planning, and participate in quality improvement activities to ensure appropriate level of care and insurance pre-certifications.

Qualifications

  • RN degree and licensure required.
  • Masters degree in a human services field with one year of relevant post-graduate mental health experience and post-graduate supervised experience in alcoholism and drug abuse counseling.

  • Review clinical documentation from referral sources to ensure appropriate admissions for level of care.
  • Coordinate bed availability and placement with unit nurses and nursing leadership.
  • Meet with patients to provide education on initial treatment planning and safety goals.
  • Prioritize bed placement based on acuity, safety considerations, and census needs.
  • Participate in quality improvement initiatives by reviewing nursing assessment documentation, patient consents, and ensuring insurance pre-certifications are completed.
  • Provide thorough reports of patient presentation and safety precautions to unit nurses to support safe transitions.

RN Case Manager: provides quality case management and qualified professional support and services to inpatient social workers as assigned; works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient’s successful inpatient treatment and discharge.

Qualifications
  • Registered Nurse degree and license
  • A graduate of a college, or university with a Masters degree in a human services field and has one year of full-time, post graduate accumulated mental health experience with the population served and a substance abuse professional shall have one year of full-time post-graduate accumulated supervised experience in alcoholism and drug abuse counseling;
