Location: West Mifflin, PA 15236

Details: Full-Time | Competitive Pay | 26 Weeks Contract with Potential to Extend

Important: Case managers will cover 5 Skilled Nursing Facilities within 20-50 miles.

Shift: 8 hrs. days, 8 am to 5 pm, (Mon to Fri)

Scheduled Hours: 40 Hours a Week

Hourly Compensation: $50.00 - $60.00 Per Hour

Job Duties:

• Ensure accurate and complete documentation and collaborate with the interdisciplinary care team.
• Assist the provider/team with care coordination activities.
• Assist with benefit determination related to Medicare parts A, B, and D.
• Collaborate with the primary advanced practice clinician (APC) and facility to respond to patient changes in condition.
• Schedule and participate in family conferences, team meetings, and case presentations.
• Coordinate training, tracking, and compliance with quality measures.
• Support services creation and delivery for targeted audiences.
• Assist Providers with documentation, coding, medication reconciliation, nursing rounds, assessments, and patient records.
• Help manage members during acute medical situations or exacerbations of chronic conditions, including testing and diagnostics.
• Support management during short-term sub-acute rehabilitation episodes.

Qualifications:

• Active and unrestricted RN license for PA or Multistate.
• Case management or care coordination experience required.
• Experience in LTC or SNF preferred.
