Job summary

Home Care with Heart is seeking a Registered Nurse Case Manager to deliver and coordinate professional nursing care in patients' homes. You will assess, plan, implement, and evaluate care plans while collaborating with physicians, nurses, therapists, and social workers to ensure high-quality outcomes.

The role covers the Trumbull County area. Active RN license, valid driver’s license, CPR, and home care experience are preferred.

Qualifications

• Active RN license required nationwide or in state of practice.
• Six months’ home care experience preferred.
• Nursing supervisory/leadership experience preferred.
• Current CPR certification and valid driver's license required.
• Ability to work flexible hours and independent
• Committed to patient-centered home health care.

Responsibilities

• Collaborate with physicians, nurses, therapists and social workers to plan and deliver home health care.
• Assess and plan patient care, direct and evaluate care plans.
• Manage emergencies promptly and maintain accurate progress notes.
• Complete medication administration and ensure regulatory compliance.
• Participate in on-call rotations and continuing education.

The RegisteredNurseCaseManager provides a wide range of nursing care to patients in the home care setting. TheRN Case Manager renders professional nursing care by assessing, developing, implementing and evaluating home care needs.

This position will serve Trumbull County. Looking for full time and part time candidates.

Responsibilities

• Work collaboratively with physicians, nurse practitioners,nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients.
• Assess, plan, direct and evaluate each patient’s care plan to ensure it meets the patient's needs.
• Manage emergency situations in a prompt, precise, and professional manner.
• Complete medication administration and treatment needs for all patients assigned.
• Provide nursing care in accordance with applicable regulations and company policies.
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis
• Provides supervision and oversight to patient care.
• Utilize the Electronic Visit Verification system.
• Prepares physician orders as applicable.
• Participates in continued education programs, patient care conferences and staff meetings as scheduled.
• Performs nursing assessments (initial and ongoing).
• Maintains excellent assessment skills and clinical knowledge and expertise.
• Notifies the physician and branch management of significant changes in patient status and obtains changes in orders as necessary.
• Assumes responsibility for ongoing interdisciplinary assessment of the patient.
• Participates in branch on-call rotation as assigned.
• Follow organizational policies and procedures regarding infection control, disposal of hazardous waste and use of protective devices as evidenced by documentation.
• Performs other job-related tasks and duties as assigned.

Qualifications

Education/Experience Desired

oNursing supervisory/leadership experience preferred

oSix months’ experience in home care preferred.

License Requirements

oValid drivers’ license

oCurrent CPR certification

SKILL REQUIREMENTS

• Ability to work flexible hours as required to meet identified patients’ needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.

• Must possess a strong commitment to the goals, mission, and philosophy of the organization.
• Understands the concepts of home health and the role of theRN as a member of a full discipline health care team.
