What You’ll Do

Strive’s RN Care Managers support patients with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). They coordinate services to optimize care, primarily via telecommunication, with possible on-site visits to healthcare settings or patient homes. This hybrid role involves in-home visits as needed and may include face-to-face meetings with colleagues and leadership. The RN Care Manager reports to the Lead, RN Care Manager.

The Day to Day
  • Develop personalized care plans addressing clinical and non-clinical needs.
  • Conduct onsite and virtual assessments to identify medical or social issues, referring matters as needed.
  • Proactively reach out to patients and clinics to evaluate status and provide assistance.
  • Manage a diverse patient panel, evaluating needs and adhering to NCQA standards.
  • Communicate updates, identify gaps in understanding, and provide education and resources.
  • Coordinate with dialysis facilities and maintain accurate records in program software.
  • Use data platforms to identify trends and areas for focus.
  • Provide culturally sensitive care and foster team collaboration.
  • Utilize clinical judgment and knowledge to deliver high-quality care.
Minimum Qualifications
  • Associate’s degree from an accredited nursing school.
  • Current RN license in the state of practice.
  • 3+ years of related education, experience, or certification.
  • Current BLS or CPR certification.
  • Reliable transportation and an active driver’s license for regional travel.
  • Ability to provide in-person patient care, including standing, sitting, walking, and lifting.
  • Internet connectivity with minimum speeds of 3.8Mbps down / 3.0Mbps up.
Preferred Qualifications
  • 3+ years in nephrology, ER, ICU, Cardiac, or ambulatory complex case management nursing.
  • Certified Case Manager (CCM) certification preferred.
  • Proficiency in Microsoft 365 Suite.

  • Experience building strong patient and family relationships.
  • Proactive communication with patients and clinics.
  • Proficiency in EMR documentation and care plan management.

Annual Salary Range: $74,000 - $90,000
