Job summary

Moments Hospice is seeking an RN Case Manager to deliver compassionate in-home care across assigned caseloads. You will assess patients, adjust care plans, and educate families while coordinating with a multidisciplinary team. A company car is provided for business and personal use with coverage of fuel and insurance.

You will work with the company owners and contribute to growth, aiming to deliver comfort in patients’ final days and support for families in our entrepreneurial culture.

Qualifications

• Licensed Registered Nurse in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and continuously refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
