Job summary

moments Hospice is seeking an RN Case Manager to provide compassionate, high-quality end-of-life care on the road in Wisconsin. You will assess patients, plan care, and coordinate with families and the healthcare team to ensure comfort and dignity.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.

Responsibilities

• Conduct initial client assessments and refine care plans.

Responsibilities:

• Conduct initial client assessments and continuously refine care plans
