Job summary

Moments Hospice in Minnesota is seeking an experienced RN Case Manager to lead symptom management and coordinate care for patients and families in the final chapter of life. You will be the clinical lead, managing assessments, developing care plans, and guiding families through end-of-life decisions.

Qualifications

• Active RN license in the state of practice.
• Proficiency with EMR systems.
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Compassionate, patient- and family-centered approach.
• Valid driver's license with reliable vehicle and current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
