StartDate: 7/14/2025 Pay Rate: $2000.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS – KY/compact RN license; AHA BLS; 2 years RN Case Management experience, with experience in a long-term acute care setting. discharge planning, clinical case management & utilization review

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Monday - Friday 8a-5p; no on-call or weekends

START DATE – 7/14/2025
