Anew Hospice - Lafayette is seeking a dedicated RN Case Manager to provide comprehensive hospice nursing care in the Lafayette, Indiana area. This full-time position emphasizes compassionate service, using advanced technology to support patients in their homes and communities.

The RN will assess, plan, implement and evaluate care, coordinating services and resources to meet individual patient and family needs. Indiana RN licensure is required; hospice or long-term care experience is preferred.

Qualifications

• Valid Indiana Registered Nurse License (RN).
• Previous hospice or long-term care experience preferred.

Responsibilities

• Delivery of patient care through the nursing process (assessment, planning, implementation, evaluation).

The Registered Nurse (RN) is responsible for the delivery of patient care through nursing process of assessment, planning, implementation, and evaluation. The registered nurse plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
