Job summary

HCA Florida Gainesville Hospital is hiring an RN Case Manager to assess patients, advocate for them within an interdisciplinary team, and coordinate comprehensive plans of care. The role emphasizes effective communication with physicians, families and external agencies, plus utilization review responsibilities.

Requirements include Florida RN license and BLS, with 3 years of acute care experience; Case Management or related certification is preferred, and BSN is advantageous as the facility

Qualifications

• RN license and BLS required; compact license holders have 60 days to obtain Florida license after relocation.
• 3 years of RN experience in an acute care setting is required.
• Certification in Case Management, Nursing or Utilization Review is preferred.

Responsibilities

• Performs a comprehensive assessment of psychosocial and medical needs of patients.
• Advocates for patients while leading the interdisciplinary team.
• Coordinates plan of care across care teams.
• Acts as liaison through effective communications with physicians, patients, families, staff, and external agencies.
• Performs utilization management reviews and communicates information to third party payors.
• Facilitates patient throughput with focus on quality and efficiency.

Job Summary And Qualifications

What you will do in this role:

• Performs a comprehensive assessment of psychosocial and medical needs of patients
• Advocates for patients while assuming a leadership role with the interdisciplinary team
• Coordinates plan of care
• Acts as a liaison through effective and professional communications with physicians, patients, families, hospital staff, and outside agencies
• Performs utilization management reviews and communicates information to third party payors
• Facilitates patient throughput with an ongoing focus on quality and efficiency

Requirements:

• Florida RN license and/or approved multistate RN license required. Compact license holders have 60 days to obtain the Florida license after relocation.
• BLS-AHA Provider issued card required within 30 days of start
• 3 years of RN experience in an acute care setting is required
• Prior experience in Case Management is preferred
• BSN is preferred
• Certification in Case Management, Nursing or Utilization Review is preferred
