Job summary

Caring Hospice Services is seeking an RN Case Manager who travels to patient locations across Massachusetts to coordinate care with families and caregivers.

The role partners with an interdisciplinary team to deliver comfort, quality of life, and resources to patients on their end-of-life journey. Massachusetts RN license in good standing is required; hospice experience preferred.

Qualifications

• Licensed as a Registered Nurse in Massachusetts in good standing.
• Associate Degree in Nursing; Bachelor's degree preferred.
• Hospice experience preferred.
• Ability to assess and respond to patient needs in various settings.
• Must be licensed and insured MA driver.

Responsibilities

• Coordinate care to ensure patients and families receive optimal support.
• Collaborate with interdisciplinary team to implement plans of care focused on comfort and quality of life.

Job Functions:

The RN Case Manager participates on the Interdisciplinary team and coordinates care that ensures patients, families and caregivers receive optimal care and resources to support their journey. The RN collaborates with other hospice staff members, volunteers, physicians and community partners to effectively implement a plan of care focuses on comfort and quality of life.

Qualifications:

• Licensed as a Registered Nurse in Massachusetts in good standing
• Associate Degree in Nursing; Bachelors degree in Nursing preferred
• Hospice experience preferred
• Skilled in nursing practice and have the ability to relate and treat terminally ill patients/families
• Ability to Assess, Adapt and respond to patient needs in a variety of settings
• Must be a license and insured driver in MA
