Job Description

The Care Manager RN leads a multidisciplinary team to deliver holistic, person-centric care management to a diverse health plan population. They serve as the single point of contact for members, caregivers, and providers via phone, email, text, and online messaging.

Using the case management process, they assess, develop, implement, monitor, and evaluate care plans to optimize health across the continuum. They collaborate with members, providers, and community resources to develop and implement care plans and achieve health goals.

Qualifications

• Nursing Diploma or Associate degree required; Bachelor’s preferred.
• 3 years clinical nursing experience in various settings.
• 1 year case management experience in managed care preferred.
• Experience managing patients telephonically and via digital channels preferred.

Licenses and Certifications

• Current, active Michigan RN license required.
• Case Management certification (CCM) required or to be obtained within 18 months.
• Chronic Care Professional (CCP) preferred.
