Job summary

HCA HealthONE Presbyterian St. Luke's in Denver, CO, is actively seeking an experienced RN Case Manager to guide patients through the care journey and coordinate services across the care team.

You will develop individualized care plans, discharge planning, and educate patients and families. A strong foundation in case management or clinical nursing is required, with a focus on compassionate, patient-centered care.

Qualifications

• 2+ years of experience in Case Management OR 3 years of experience in clinical nursing required.
• RN licensure is required.

Responsibilities

• Guide patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths, needs, and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations, documenting findings and communicating needs across the care team.
• Collaborate with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA HealthONE’s values of compassion, respect, and excellence in every patient and family interaction.

Job Summary and Qualifications

As a Case Manager, your rolewill be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.

Your responsibilities will include:

• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care

What qualifications you will need:

• (RN) Registered Nurse


• 2+ years of experience in Case Management OR 3 years of experience in clinical nursing required
