Job summary

CARIN Alliance is seeking an RN Case Manager to provide end-of-life hospice care in patients' homes or community settings. The role involves supervising hospice aide staff, coordinating patient services, and collaborating with an interdisciplinary team.

Requirements include active RN licensure in the state, a valid driver’s license, and a minimum of two years of RN experience (hospice experience preferred).

• At least two years of RN experience, with hospice or palliative care experience preferred.
• Strong clinical judgment, excellent communication, compassion, and ability to handle sensitive situations.
