Position Purpose

An exceptional opportunity for a Registered Nurse Case Manager to support our mission as a compassionate and transformative healing presence in our community. We value diversity, believing it makes us stronger.

The case manager coordinates healthcare plans for assigned patients through care plans, pathways, managed care, and utilization review, collaborating with the healthcare team. Responsibilities include evaluating care quality, access, and cost-effectiveness, maintaining care continuity, and providing age, cultural, and spiritual sensitive care. The role involves cross-training in Emergency, Inpatient Rehab, and Inpatient settings.

Shift and Status

Shift: Days/Afternoons

Status: Full-time, benefit-eligible (80 hours per pay period, every two weeks)

Required Qualifications

• Bachelor's Degree in Nursing
• Three to five years of related experience and relevant knowledge of patient populations
• Current RN License in Michigan; CCM certification required within 2 years of eligibility
