The case manager coordinates healthcare plans for assigned patients through care plans, pathways, managed care, and utilization review, collaborating with the healthcare team. Responsibilities include evaluating care quality, access, and cost-effectiveness, maintaining care continuity, and providing age, cultural, and spiritual sensitive care. The role involves cross-training in Emergency, Inpatient Rehab, and Inpatient settings.
Shift: Days/Afternoons
Status: Full-time, benefit-eligible (80 hours per pay period, every two weeks)
