Job summary

Harmony Hospice Care is seeking a Registered Nurse (RN) Case Manager to serve Akron/Canton and surrounding areas. The RN Case Manager will support the team with case management visits and care coordination, ensuring continuous, well-coordinated care.

The ideal candidate holds a current Ohio RN license, hospice experience, and is willing to travel. Monday through Friday 8:30am–4:30pm, with a competitive salary and generous benefits including 401k and PTO.

Qualifications

• Current Ohio RN license in good standing.
• Hospice experience preferred.
• Willingness to travel within the Akron/Canton area.

Responsibilities

• Perform initial patient and family assessments to determine hospice care needs.
• Regularly reevaluate patient nursing needs.
• Use assessment data to determine care plans.
• Collaborate with the healthcare team, patient, and family to coordinate care.
• Counsel patient and family on end-of-life needs.
• Initiate appropriate preventive nursing procedures.
• Communicate patient needs and changes in condition with the physician.
• Supervise Hospice Aides and LPNs.

• REGISTERED NURSE - RN - CASE MANAGER - HOSPICE - Akron/Canton and surrounding area : Harmony Hospice Care is looking to hire a 'Registered Nurse (RN) Case Manager to join our team of professionals for Akron/Canton and the surrounding areas. The RN Case Manager will be responsible for assisting the Team with Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Registered Nurse Supervises the Hospice Aides and LPN's

The proper candidate must have a current Ohio Registered Nurse RN License in good standing, must be familiar with and willing to travel the area and have hospice experience. Must also have reliable transportation and insurance. Monday through Friday 8:30am to 4:30pm. Care for Hospice patients in their homes and facilities.
