Qualifications

• Minimum five (5) years’ experience in Clinical Nursing.
• Minimum one (1) year experience in case management, preferably in a hospital setting.
• Supervisory experience, preferred.
• Critical thinking, service excellence and good interpersonal communication skills.
• Capacity to learn, synthesize, and make critical judgments.

Responsibilities

• Assists in development and implementation of the case management program aligned with strategic goals.
• Oversee staff including screening, training, performance management, and disciplinary support.
• Coordinate care with other providers and community resources for seamless care.
• Provide comprehensive reporting including KPI's, avoidable delays, LOS and other metrics.
• Ensure compliance with applicable regulations and participate in quality improvement.
