Qualifications

• RN or Social Worker required; PA RN license with BSN or MSW degree.
• Three years experience in acute care or health insurance environment.
• Three years experience with physicians, staff and executives.
• Two years leading or managing others.
• Certification in Case Management or must obtain within one year of hire.

Responsibilities

• Provide clinical oversight to case management staff and coordinate daily operations.
• Assist clinical teams with processes and strategic projects.
• Promote staff development and maintain education requirements; oversee care coordination activities.
• Assess staff competency and manage performance records and reviews.
• Assist Director with budget needs and data quality monitoring; prepare reports.
• Set quality and productivity standards and take corrective actions as needed.
