Job summary

Reston Hospital Center is seeking a Case Manager for the Rehab Unit to coordinate rehabilitative care and optimize patient outcomes. The CM will work with the Rehab Program Director and care team to plan and direct care tailored to each patient’s needs.

The role requires an RN with at least an Associate Degree and 1 year of rehab experience; BLS within 30 days is needed. Join a team dedicated to compassionate, patient-centered care at our Reston facility.

Qualifications

Responsibilities

• Guide patients and families through program orientation, discharge criteria, and understanding Medicare/insurance benefits.
• Coordinate education for patients, families, and caregivers to support treatment planning.
• Develop and implement individualized treatment plans reflecting patient goals and needs.
• Complete psychosocial assessments and discharge planning evaluations; document findings clearly.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless care.
• Promote HCA Healthcare values in every patient and family interaction.

Job Summary And Qualifications

The Case Manager (CM) ensures high quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes. As an active member of the rehab team, the CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions, and coordinates efforts within the rehabilitation team.

Your responsibilities will include:

• Guiding patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
• Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
• Promoting HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• (RN) Registered Nurse
• Associate Degree
• at least 1 year of experience in Rehab
• prefer previous experience in case management
