We are searching for a talented RN-Case Coordinator
Hours:Per Diem
Shift: Day shift with weekend and holiday rotation; 7:30am - 4:00pm
Location: Charlton Memorial Hospital - Fall River, MA

Under direction of the Manager, Case Coordination, perform all the functions of a case manager. Coordinates the care/services of assigned patients along the continuum of care. Works collaboratively with the physician(s) and multidiscipline team members both internal and external to the organization to improve patient care through effective utilization and monitoring of health care resources. Facilitates patient/family/representative achievement of desired outcomes. This position is part of Core Case Management staffing. It supports utilization management functions including physician to physician consultation for correct level of care determinations. This position also supports denial prevention and compliance with COP requirements.
Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
Compensation: Pay rate will be determined based on level of experience.
