Job summary

Carle Health is seeking a Registered Nurse in Peoria, IL to oversee clinical and financial outcomes during acute hospitalizations. You will assess patient needs, coordinate discharge planning, and lead the five components of case management to ensure safe transitions from the hospital to post-acute settings.

Responsibilities include collaboration with physicians, social workers, and care teams to optimize length of stay, identify post-acute needs, and monitor progress toward patient outcomes

Qualifications

• Licensed RN in Illinois (IDFPR).
• Bachelor's Degree in nursing or related field.
• Experience in hospital-based case management preferred.

Responsibilities

• Liaises with patient/family and physician to determine next level of care and manage LOS.
• Coordinates transition from inpatient to post-hospital care with internal/external referrals.
• Documents plan of care and utilization issues in case management systems and care plans.
• Evaluates plan of care effectiveness and collaborates with physicians to reduce readmission risk.
