Carle Health is seeking an experienced Registered Nurse for case management during acute illness requiring hospitalization. You will oversee initial assessment of needs, ongoing evaluations, and discharge planning to ensure a safe and timely transition from the hospital to appropriate post-acute care.

Qualifications

• Licensed RN in Illinois (IDFPR) with nursing degree.
• Bachelor's degree required; licensure to practice.
• Experience coordinating transitions from inpatient to post-hospital care with physicians, consultants, and social workers.

Responsabilités

• Assess and plan post-hospital care within 24-48 hours of admission and throughout the stay.
• Coordinate transition from inpatient care to post-hospital care with the healthcare team.
• Document plan of care and utilization issues in case management software and multidisciplinary notes.
• Evaluate plan of care effectiveness and monitor readmission risk and LOS.
