Job summary

Option Care Health is seeking a Clinical Transition Specialist/Registered Nurse in Spokane to educate patients and caregivers on home infusion services and ensure smooth transitions to alternative sites. The role focuses on building referral partnerships and achieving sales goals while coordinating with hospital staff and internal teams.

The position requires an RN license, 2+ years in healthcare, strong relationship skills, and travel to care facilities.

Qualifications

• Licensed Registered Nurse (RN) in the practicing state with at least 2 years in healthcare.
• Experience establishing and maintaining relationships with individuals at all levels of the organization and with vendors.
• Basic PC skills: Internet, email, Microsoft Word/Excel/PowerPoint.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Educates patients, families and facility staff about Option Care services for discharge planning.
• Coordinate with intake, case managers, discharge planners to ensure seamless transitions to home or ATS.
