Qualifications

• RN license in Ohio and Indiana.
• Minimum of 2 years healthcare experience, preferably in home health or sales/marketing.
• Strong communication, assessment, and decision-making skills.

Responsibilities

• Collaborate with hospitals, nursing homes, and home health personnel to ensure quality care.
• Meet or exceed monthly admission targets as provided by supervisor and budget.
• Coordinate patient referrals and arrange home health services.
• Educate caregivers about home health options, Medicare/Medicaid guidelines, and insurance benefits.
• Develop relationships with healthcare providers to promote home health services.
• Participate in provider education and support hospital staff as a home health resource.
• Conduct trend analysis and identify opportunities for improving home health outcomes.
• Contribute to Quality Assurance Performance Improvement initiatives.
