Responsibilities

The Care Transition Coordinator’s primary responsibility is to facilitate a seamless transition for patients discharging from a facility to hospice care.

• Responsible for achieving admission goals as established at hire or during annual review.
• Assist the agency with preparing for and accepting patient care post-discharge.
• Assist with executing contracts for facility-based services.
• Explain hospice services and procedures to patients and families.
• Involve families and caregivers in education and assess post-discharge educational needs.
• Participate in bi-weekly IDG meetings to update on accounts, needs, and growth strategies.

Education and Experience

• One year hospice or hospital case management experience.
• Current RN, LPN, or SW licensure in the state of practice.
• Excellent organizational, verbal, and written communication skills.
• Thorough understanding of hospice criteria and coverage guidelines.
• Proficient computer skills.
• Current CPR, valid driver's license, vehicle insurance, access to a dependable vehicle or public transportation.
