Job summary

University of Utah Health Hospitals and Clinics is seeking a qualified nurse coordinator to manage patient care plans, coordinate interdisciplinary activities, and ensure smooth discharge planning at Huntsman Cancer Hospital. The role requires RN licensure and related healthcare coordination experience, with a day shift and fixed schedule.

The coordinator will work with a 24/7 health system, attending rounds and conferences, and will support staff through care planning and post-discharge

Qualifications

• Bachelor's Degree in Health Care Administration, Health Science or related field or equivalency.
• One year of related nursing experience.

Responsibilities

• Assesses and coordinates patient/family care needs throughout the patient’s stay.
• Acts as the key coordinator and contact for the patient care plan.
• Addresses complex patient needs and supports bedside staff with complex procedures.
• Collaborates with Care Coordinators and Charge Nurses to identify burden of care and assign core team members.
• Participates in medical rounds at least two mornings a week.
• Attends evaluation conferences twice weekly and communicates progress and discharge barriers.
