Job summary

Independent Living Systems (ILS) is seeking an RN Care Manager to coordinate care for Medicaid and Medicare Special Needs Programs in Florida. You will assess needs, develop person-centered plans, and collaborate with providers, families, and community resources to ensure optimal outcomes.

Responsibilities include care plan development, progress monitoring, documentation, and ensuring adherence to program requirements while advocating for patients and promoting independence and quality of life.

Qualifications

• Florida-licensed RN with 2+ years caring for disabled and elderly populations.
• Experience in care management or care coordination.
• Proficiency with electronic health records (EHR).

Responsibilities

• Assess member needs and identify care gaps to develop a person-centered plan.
• Coordinate access to services across the care continuum.
• Monitor progress and adjust plans to evolving needs and goals.
• Collaborate with providers, families, and community resources for seamless care.
• Provide education and support on resources, treatment adherence, and self-advocacy.
• Maintain accurate documentation and ensure compliance with policies and standards.
• Stay informed about current care trends and program requirements.
• Perform other duties as assigned.
