Job summary

CareSource/Commonwealth Care Alliance (CCA) seeks an Integrated Care Clinical Manager to monitor and manage care for dually-eligible enrollees with complex needs. The role focuses on integrating health services and resources to coordinate care and improve outcomes.

The RN will lead an interdisciplinary team, perform comprehensive assessments, and advocate for enrollees while coordinating with providers and community agencies. Travel is required within MA to support member care.

Qualifications

• RN with an AS in nursing from an accredited program.
• Registered Nurse able to serve people with complex medical, behavioral, and social needs.

Responsibilities

• Engage enrollees in homes and community settings to establish complex care management relationships.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Coordinate with providers and community resources to ensure seamless care transitions.
