Qualifications

Responsibilities

• Utilization Management and care transitions, coordinating documentation and authorizations
• Review admissions within 24 hours against high-risk criteria
• Complete assessments on readmissions within 30 days and provide data
• Perform daily utilization reviews for medical/psychiatric appropriateness
• Discharge planning rounds and collaboration with multidisciplinary team
• Assist with documentation and reporting as required by department
