Qualifications

• Graduate from a recognized, accredited school of nursing (Required).
• 3 years of clinical experience in chronic disease states, with demonstrated clinical competence (Required).
• RN - Registered Nurse with Georgia licensure and/or multistate licensure Current licensure to practice Nursing in Georgia (Upon Hire Required).

Responsibilities

• Coordinate medical home and care coordination across the health care continuum for all patients in the physician office setting.
• Ensure safety, best practice and high quality standards of care across the continuum.
• Coordinate self-management support and disease registry activities for the clinic's patient population.
• Assist in routing patients to complementary support services and education/outreach programs.
