Job summary

Talented Medical Solutions seeks an experienced Registered Nurse to support pre-visit planning and patient care coordination in Oklahoma. You will communicate patient needs to the staff and maintain the team’s patient panel.

Identify high-risk sub-populations for care management, track monthly performance measures, and participate in continuous quality improvement using the Model for Improvement while ensuring CN documentation and patient education.

Qualifications

• Minimum 3 years nursing experience.
• Oklahoma RN license or Compact/multi-state license.
• Certs: CPR, BLS, and ACLS.
• Excellent written and verbal communication skills.
• Pre-employment credentialing checks (Background Check, Drug Screen, etc.).
• Ability to meet physical demands (stand; walk; sit; lift up to 50lbs).

Responsibilities

• Does pre-visit planning for scheduled patients.
• Communicates patient needs and recommended treatment to staff on their team.
• Maintains & updates the team's patient panel.
• Identifies, creates, & maintains panels of high risk sub populations for care management.
• Tracks performance measures monthly and shares with the team, staff and patients.
• Participates in ongoing quality improvement activities using the Model for Improvement.
• Contacts patients for follow up diagnostic tests, referrals, appointments, or medical follow up as appropriate.
• Schedules nursing visits for follow up.
• Tracks and follows up on referrals.
• Provides patient education and self-management support.
• Triages patients on the team's panels who call or walk in and directs them appropriately.
• Documents observations, assessments, nursing interventions, treatments, and referrals according to CN documentation standards.
• Provides direct nursing care back up as the team needs dictate.
