UChicago Medicine AdventHealth Bolingbrook has been providing faith-based care in the Adventist tradition to Chicago’s western and southwestern suburbs since 2008. The 138-bed medical center features an award-winning Level II trauma center and offers the latest advancements in medical technologies and care with private, family-friendly patient rooms and access to top doctors.

The RN Care Manager collaborates with patients, families, social workers, nurses, physicians, and the interdisciplinary team to ensure patient-centered care coordination and progression through the continuum of care. Responsibilities include developing and implementing transition of care plans, facilitating high-quality patient care, and ensuring safe and smooth transitions post-hospitalization.

Qualifications include an Associate Degree in Nursing or RN Diploma, current Illinois RN license, and at least two years of hospital nursing experience. Preferred qualifications include a BSN, MSN, or prior care management experience.

This is a per diem position with a schedule of at least 2 weekend days per month and rotating holidays. Pay ranges from $35.18 to $62.17 per hour.
