Qualifications

• Bachelor's degree in Nursing strongly preferred; required in some locations (Sioux Falls).
• Graduate from an accredited nursing program (CCNE, ACEN, or NLN CNEA).
• Minimum of two years’ clinical experience preferred.
• Current RN license in the state or multistate license under NLC.
• Basic Life Support (BLS) certification required within six months of employment.
• Valid driver's license.

Responsibilities

• Provide individualized nursing care using the nursing process (assessment, diagnosis, planning, implementation and evaluation).
• Educate patients based on assessed needs and coordinate care across hospital and clinic settings.
• Collaborate with inter-professional team and community partners to manage patient care across the continuum.
• Lead and coordinate care and participate in population health initiatives for panels of patients.
