• Minimum 1 year of care management or utilization review experience, or completion of TIPS/Case Management Orientation.

Responsibilities

• Carry out effective case management functions for identified patients, coordinating and managing care across the continuum.
• Communicate daily with medical staff and outside resources to ensure appropriate utilization of resources according to clinical and financial standards.
