Habitat Health is seeking an experienced registered nurse to deliver direct clinical care across clinics, adult day centers, homes, virtually, and SNFs as part of the interdisciplinary team. The role emphasizes care coordination, patient assessments, and personalized care plans for a panel of participants.
Ideal candidates hold an RN license (BSN preferred) with 2–4 years of geriatric experience and case management skills.

In this role, you willbe responsible forthe management and delivery of direct nursing care to our participants in a variety of settings including, but not limited to, the clinic, adult day center, participant’s home, virtually, and SNF. Youwill serve as a critical member of the Interdisciplinary Team (IDT) and work collaboratively to complete assessments and drive forward participant care plansas “quarterback” for your panel.
Wetake into accountan individual’s qualifications, skillset, and experience in determiningfinalsalary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is$55.00to $68.00/hour. The actual offer will be at the company’s sole discretion anddeterminedby relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
