Job Overview

The RN Care Manager is responsible for assessing the care needs of participants, providing nursing interventions, and evaluating care outcomes. Collaborates with the interdisciplinary team to develop and implement care plans, delegates tasks to aides, and provides care in clinics, centers, and homes as needed.

Compensation

This range is provided by Beacon of LIFE (A PACE Program). Your actual pay will depend on your skills and experience. Base pay range: $41.00/hr - $52.44/hr.

Responsibilities

• Provide high-quality clinical care as part of the interdisciplinary team.
• Conduct assessments, develop care plans, and monitor participants’ health needs.
• Participate in on-call triage and coordinate home care services.
• Ensure timely follow-up on medical visits and test results.
• Reconcile medications and educate participants and caregivers.
• Develop and revise care goals collaboratively.
• Delegate tasks to aides and evaluate their competence.
• Participate in team meetings, quality improvement, and training.

Qualifications

• BSN preferred; RN license required.
• Minimum 1 year experience as an RN, preferably with elderly or frail populations.
• Experience in home care, long-term care, or managed care is a plus.

Monday - Friday, 8am - 4:30pm
