Job summary

Aetna is seeking a Care Manager Registered Nurse to support members enrolled in Medicare and Medicaid. You will use clinical judgment to drive appropriate healthcare outcomes and assist with scheduling and benefits access.

Ideal candidates have 3–5 years of clinical experience, an Illinois RN license, and experience in care management or discharge planning. This is a full-time role based in Champaign, IL with significant travel up to 50-75%.

Qualifications

• Associate's degree required; bachelor's degree preferred.
• Minimum 3–5 years clinical practical experience.
• 2–3 years care management, discharge planning and/or home health care coordination experience preferred.
• Active and unencumbered RN license in Illinois required.
• Certified Case Manager preferred.

Responsibilities

• Collaborate and inform the case management process.
• Facilitate appropriate healthcare outcomes for members.
