Responsibilities

• Collaborate with multidisciplinary team to develop and report the case management program.
• Act as liaison among patient, family, nurses and physicians to optimize outcomes.
• Coordinate continuum of care from pre-admission through post-discharge.
• Ensure the interdisciplinary care plan aligns with the patient’s clinical course and services.
• Discuss resource utilization and discharge planning with physicians.
