Geisinger at Home is seeking an experienced RN to provide in-home care for Medicare Advantage members across multiple counties. You will work with physicians and a multidisciplinary team to implement individualized care plans and coordinate discharge planning, ensuring quality outcomes.

The role involves typical weekday hours with one weekly later shift and rotating weekends/holidays. A strong emphasis on collaboration and patient-centered care is central to this position.

Competenze

• At least 3 years of RN work experience required.
• Experience in case management, critical care, emergency, or home healthcare is a plus.

Mansioni

• Develop plan of care for each patient from admission through discharge.
• Monitor patient care through assessments and evaluations.
• Coordinate efforts of the care team and facilitate discharge planning.
• Ensure adherence to quality standards and data entry for quality improvement.
