Job summary

Hutchinson Regional Healthcare System in Hutchinson, KS, is seeking a full-time RN Care Manager to coordinate care for adult patients in an interdisciplinary environment. You will gather data, monitor care delivery, and perform utilization reviews while supporting rehab education for staff.

The role requires completion of an approved nursing program and an active RN license in Kansas; BSN is preferred. Excellent communication and collaboration skills are essential.

Qualifications

• Ability to develop patient plan of care.
• Demonstrated ability to communicate effectively.
• Ability to critically think through a patient care needs.
• Completion of an approved school of nursing program.
• One year experience as a registered nurse.
• Current licensure as an RN in the State of Kansas.
• BSN preferred.
• Care management experience preferred.

Responsibilities

• Shall be responsible for providing care coordination including needs assessments and identification of care options, services, communication with patients and families in an interdisciplinary environment.
• Gathers and manages patient data related to case management, care planning, and QI analysis.
• Monitors delivery of care through patient rounds, documentation, identifying progress toward desired outcomes, maintaining ongoing communication with utilization review staff, etc.
• Performs utilization review for all patients.
• Able to assist nursing staff with patient care when needed.
• Assist in Rehab training and education to staff when needed.
• Complies with hospital and legal requirements by fostering nursing practices that adhere to the philosophy, goals, and standards of care.
• Abides by the Health Insurance Portability and Accountability privacy and security regulations regarding all aspects of Protected Health Information (PHI).
• Other duties as assigned by Director
