Job summary

Adventist Health White Memorial in Los Angeles, CA seeks an RN for a part-time Per Diem Case Management role. You will coordinate a continuum of care from pre-admission through post-discharge, liaising with patients, families, and the care team to optimize outcomes and ensure timely, appropriate care.

Requires RN licensure and preferred BSN; prior care management experience is a plus. Per Diem position with base pay in a comprehensive rewards package including benefits.

Qualifications

• Bachelor's Degree in Nursing (BSN): Preferred.
• Experience in a care management role: Preferred.
• Registered Nurse (RN) licensure in the state of practice: Required.
• Hospital Fire and Life Safety (HLFS): Required.

Responsibilities

• Leads the coordination of patient care with other disciplines within the care team, monitoring the appropriateness and timeliness of care.
• Ensures the interdisciplinary care plan is consistent with the patient’s clinical course and continuing care needs.
• Discusses with physicians the appropriateness of resource utilization, consultations, treatment plan, LOS, and discharge plan.
• Coordinates transfer of patients to tertiary centers and transfer of patient information for continuity of care.
• Provides oversight and collects data required for regulatory and accreditation compliance.
• Performs other job-related duties as assigned.
