Job summary

You will collaborate with interdisciplinary teams, review records for medical necessity, and ensure compliance with regulatory standards while maintaining thorough documentation and patient-centered care.

Qualifications

• Associate Degree in Nursing is required.
• 2-4 years of clinical nursing experience in a hospital, home health, or nursing home setting is required.
• 2-4 years of care management experience preferred.
• ACM/ CCM certification preferred.

Responsibilities

• Conduct daily reviews of medical records to assess admission necessity and utilization.
• Collaborate with interdisciplinary teams to coordinate patient care and transitions.
• Develop and implement discharge plans including post-hospital placement and social services.
• Refer cases to physicians or managers when criteria are not met for timely interventions.
• Maintain accurate documentation of case management activities and referrals.
