Lower Keys Medical Center is seeking a Behavioral Health Care Manager RN to oversee discharge planning, transitions of care, and case management. The role collaborates with interdisciplinary teams to ensure appropriate utilization of services and compliant care.

The RN will review medical records, develop discharge plans, coordinate post-hospital placements, and maintain thorough documentation while meeting regulatory standards.

Qualifications

• ADN required; BSN preferred.
• 2-4 years of clinical nursing experience in hospital/home health/nursing home.
• 2-4 years of care management experience preferred.

Responsibilities

• Oversees discharge planning and transitions of care.
• Reviews medical records for appropriateness and necessity.
• Collaborates with IDT to coordinate patient care.
• Develops and implements discharge plans and post-hospital placement.
• Maintains timely documentation and compliance with standards.
• Assists patients, families, and physicians with post-hospital options.
