Qualifications

Position Summary: $10,000 Sign-on Bonus (pro-rated for part time and per diem). As an integral member of the Project SHINE clinical care management team, the Registered Nurse (RN) Care Manager (CM) will have the opportunity to make a profound impact on the lives of people living with HIV/AIDS and those at risk for HIV transmission, many of whom face multiple barriers that hinder self-care. The role involves flexibility, working with high-need communities, and may include telephonic or in-person outreach in various settings such as the health center, community, home, or inpatient facilities.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  1. Demonstrates basic to intermediate proficiency in HIV-specific nursing care.
  2. Completes nursing intakes, assessing patients through history, tests, and observations.
  3. Provides thorough clinical follow-up, addressing care gaps, vaccinations, and screenings.
  4. Works with the healthcare team to develop individualized care plans, including adherence strategies.
  5. Assumes a leadership role within the multidisciplinary team.
  6. Promotes teamwork, demonstrates self-direction, problem-solving, and provides leadership to support staff.
  7. Interacts professionally and respectfully with patients and staff, maintaining a positive attitude.
  8. Educates patients on chronic disease management, adherence, STI screening, and safe sex.
  9. Knowledgeable about substance abuse, mental health, and other conditions; sets appropriate limits.
  10. Implements HIV nurse-on-call procedures, acting as liaison to inpatient facilities, physicians, and mental health staff.
  11. Performs safe triage, administers medications, tests, and treatments as ordered, managing refills with provider authorization.
  12. Documents patient histories, assessments, plans, treatments, and communications thoroughly in the electronic health record.
  13. Follows policies on domestic violence screening, safety, and infection control; participates in programs and committees.
  14. Utilizes training to enhance clinical practice and disseminates health education.
  15. Participates in grant writing and reporting related to nursing services and patient outcomes.
  16. Engages in quality improvement initiatives related to nursing.
  17. Attends departmental meetings and adapts to changing needs and schedules.
  18. Performs other duties as assigned, including STI assessments, referrals, and coordination of HIV prevention and treatment programs.
  19. EDUCATION:

  • High School Diploma required.
  • Vocational/technical training in an accredited nursing program required.
  • Massachusetts licensure as an R.N. or L.P.N. required.
  • BCLS certification required.

EXPERIENCE:

  • Preferred: Two years in HIV nursing, preferably in community health.
  • Experience with substance abuse and HIV care patients preferred.

SKILLS/ABILITIES:

  • Spanish bilingual/bicultural skills preferred.
  • Excellent interpersonal skills.
  • Proficient in Microsoft Word.
  • Flexible hours, including weekends and evenings; 24-hour on-call rotation.
