Position Summary: $10,000 Sign-on Bonus (pro-rated for part time and per diem). As an integral member of the Project SHINE clinical care management team, the Registered Nurse (RN) Care Manager (CM) will have the opportunity to make a profound impact on the lives of people living with HIV/AIDS and those at risk for HIV transmission, many of whom face multiple barriers that hinder self-care. The role involves flexibility, working with high-need communities, and may include telephonic or in-person outreach in various settings such as the health center, community, home, or inpatient facilities.
